Family Membership and Donation Form

Our family membership consists of the children, spouses, grandchildren, and connections of the ancestors/branches of Alfred & Hannah Wesley of this family generation.  Dues are collected bi-annually (on years that the Family Reunion does not have a reunion, for example, 2024, 2026, 2028).  Membership Dues are $15.00 per adult (age 25 and older). The deadline for making payments is July 31, 2024, of the payment year. Please complete the form below, and you will be directed to the payment page or download/print the form to be mailed with your payment.  If you will be paying dues for more than one family member, please list their names in the provided space below so that they may receive proper credit.  (If you are making a donation only, please complete the form below and then choose the donation option on the payments page).
