Please complete this form for each deceased family member(s) that passed between July 1, 2023 – July 1, 2025, with as much information as you know. Special recognition will be given to our deceased family members during the luncheon.  Also, if you have a picture of your loved one, please send it as well.

Your information must be received by Monday, July 1, 2025, for your loved one to be recognized during the reunion's Memorial Service.

If you are not able to submit the form prior to the reunion, please bring it to turn in at the Meet and Greet, Thursday July 17, 2025.
